Key Responsibilities
- Provide physical therapy services to patients in a home health setting
- Meet productivity expectations of 6 points per day or 30 points per week
- Complete and maintain accurate clinical documentation including OASIS assessments
- Adhere to dress code requirements of business casual attire or solid color scrubs
Required Qualifications
- Active Physical Therapist license valid in Virginia (in hand at time of submittal or proof of compact license)
- Minimum 1 year of home health experience required
- Proficiency in OASIS documentation
- Valid SSN required at time of submittal
- One supervisory reference required at time of submittal
Preferred Experience / Skills
- Experience with electronic health record systems
- Strong time management skills with ability to maintain consistent productivity standards
