    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities.
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
    • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing.
    • Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations.
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Connecticut.
    • Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans.
    • Proficiency in clinical documentation in compliance with state, federal, and payer guidelines.
    • Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct.
    • Commitment to patient and employee safety standards and safe patient handling practices.
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting.
    • Experience supervising or mentoring physical therapy students.
    • Familiarity with performance improvement methodologies and quality initiatives.
