Key Responsibilities

• Provide physical therapy evaluation and treatment services to residents in a skilled nursing facility setting
• Deliver care to both long-term care and short-term rehabilitation patients
• Develop and implement individualized treatment plans in accordance with physician orders and clinical best practices
• Document patient progress, treatment plans, and outcomes in compliance with facility and regulatory standards
• Collaborate with interdisciplinary team members to support patient goals and transitions of care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Maryland
• Graduate of an accredited Physical Therapy program
• Expected 40 hours per week availability

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care environment
• Experience with short-term rehabilitation populations
• Familiarity with electronic medical records and therapy documentation systems
