Physical Therapist (PT) – Home Health

Vernon, CT

Schedule / Job Type

Contract | Full-Time | Days | Monday – Friday with weekend rotation (approximately every 6 weeks)

Key Responsibilities

• Perform Start of Care evaluations for home health patients within an assigned territory
• Deliver skilled physical therapy services to patients in their homes within an approximate 40-mile radius

• Meet productivity expectations of 28–30 points per week
• Document all patient care activities accurately using HCHB electronic medical records
• Oversee and direct the work of Physical Therapist Assistants (PTAs) as applicable
• Participate in weekend rotation coverage as scheduled

Required Qualifications

• Active Physical Therapist (PT) license in the state of Connecticut
• Prior home health experience required
• Experience with HCHB (Homecare Homebase) EMR system required
• OASIS experience required
• Ability to travel within an approximate 40-mile radius of the assigned territory

Preferred Experience / Skills

• Experience supervising or directing Physical Therapist Assistants (PTAs)
• Strong organizational and time management skills to support productivity goals in a home health setting
