Evaluate patients and develop individualized physical therapy treatment plans
Deliver hands-on therapeutic interventions in a post-acute care setting
Monitor and document patient progress, adjusting plans of care as appropriate
Collaborate with interdisciplinary team members to support patient recovery and discharge goals
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
Active California Physical Therapist (PT) license required
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Preferred Experience / Skills
Prior experience in a post-acute, skilled nursing, or long-term care setting
Familiarity with Medicare documentation and compliance requirements
