Evaluate patients and develop individualized physical therapy treatment plans
Deliver skilled physical therapy interventions in a post-acute care setting
Monitor and document patient progress toward established functional goals
Collaborate with interdisciplinary care teams to coordinate patient care
Educate patients and families on therapeutic exercises and home programs
Required Qualifications
Active California Physical Therapist (PT) license in good standing
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Preferred Experience / Skills
Prior experience in a post-acute, skilled nursing, or rehabilitation setting
Strong documentation and clinical assessment skills
Ability to work effectively within a multidisciplinary team environment
