Contract | Full-Time | Onsite | Day Shift | 5x8-hour shifts, 40 hours per week

• Provide direct physical therapy services to patients in accordance with established care plans and clinical standards
• Evaluate patient conditions, develop individualized treatment plans, and monitor progress toward therapeutic goals
• Deliver hands-on therapeutic interventions appropriate to the clinical setting
• Maintain accurate and timely clinical documentation in compliance with facility and regulatory requirements
• Collaborate with interdisciplinary care teams to ensure coordinated and effective patient outcomes

• Active Physical Therapist license (must be valid and in good standing at time of submission)
• Relevant clinical experience in a physical therapy setting
• Ability to meet credentialing requirements for contract quality assurance review
• Must provide proof of license application within 5 business days of booking if licensure is pending

• Prior experience working in a similar outpatient or facility-based physical therapy environment
• Strong communication and documentation skills with experience in multidisciplinary team settings
