Key Responsibilities
- Evaluate and treat patients in an outpatient physical therapy setting
- Develop and implement individualized treatment plans based on patient needs and clinical findings
- Document patient progress and maintain accurate clinical records in accordance with facility requirements
- Collaborate with the interdisciplinary care team to ensure optimal patient outcomes
- Adhere to all credentialing, compliance, and documentation requirements as outlined by the facility
Required Qualifications
- Active Physical Therapist license required; must obtain permanent Arizona state licensure promptly upon assignment start
- Prior experience in an outpatient physical therapy setting
- Ability to meet facility-specific credentialing and quality assurance requirements
- Compliance with all facility onboarding and documentation processes prior to start date
Preferred Experience / Skills
- Previous experience working in a high-volume outpatient therapy environment
- Familiarity with outpatient documentation systems and clinical workflows
