Key Responsibilities
Manage an average of 2–5 patient visits per day, including evaluations, start-of-care assessments, and subsequent visits
Meet a weekly productivity expectation of 25 points per week
Document all patient care using the HCHB Pointcare electronic medical records system
Provide supervision for 1–2 Physical Therapist Assistants (PTAs)
Travel to patient homes within the East Portland coverage area
Required Qualifications
Active Physical Therapist (PT) license in the state of Oregon
Experience in a home health setting
Ability to travel throughout the East Portland service area
Ability to complete orientation requirements, including compliance training and ride-alongs
Preferred Experience / Skills
Prior experience with HCHB Pointcare EMR system
Experience supervising PTAs in a home health environment
