Schedule / Job TypeContract | Full-Time | Onsite | Day Shift | 5 x 8-hour shifts per week, 40 hours per week
Key Responsibilities- Provide direct physical therapy services to patients in an outpatient therapy setting
- Evaluate, assess, and develop individualized treatment plans based on patient needs and goals
- Implement evidence-based therapeutic interventions to improve patient mobility, strength, and function
- Complete accurate and timely clinical documentation in accordance with facility and regulatory requirements
- Collaborate with interdisciplinary care teams to coordinate patient care and progress
Required Qualifications- Active Physical Therapist license in the state of Indiana (permanent license required)
- Graduate of an accredited Physical Therapy program
- Prior experience in an outpatient physical therapy setting
- Ability to meet credentialing and compliance requirements prior to assignment start
Preferred Experience / Skills- Experience working in a high-volume outpatient therapy environment
- Strong documentation skills with familiarity in electronic medical records systems
