Key Responsibilities
- Provide direct physical therapy services to patients in an outpatient clinical setting
- Evaluate, assess, and develop individualized treatment plans for patients
- Deliver therapeutic interventions in accordance with established clinical protocols and standards of care
- Complete required clinical documentation accurately and in a timely manner
- Collaborate with interdisciplinary care teams to support optimal patient outcomes
Required Qualifications
- Active Physical Therapist license — permanent Indiana state license required; compact license not accepted
- Prior experience as a Physical Therapist in a relevant clinical setting
- Ability to meet facility-specific credentialing and contract QA requirements prior to start
- Willingness to complete any facility-required documentation outside of standard onboarding clearance processes
Preferred Experience / Skills
- Experience in an outpatient physical therapy environment
- Familiarity with outpatient documentation systems and clinical workflows
