Key Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs with the primary goal of restoring function and preventing or minimizing disability following disease or injury
- Conduct initial admission and evaluation visits, and notify the assigned Clinical Team Manager of OASIS scores within established timelines
- Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans
- Help clients reach their maximum performance and functional level within the limits of their capabilities
- Supervise Physical Therapy Assistants as applicable
- Perform other duties consistent with skills, training, and the goals of the position
Required Qualifications
- Active Physical Therapist license valid in the state of California
- Minimum of one year of Home Health experience
- Proficiency in evaluating and treating patients under medically prescribed physical therapy programs
- Ability to complete and submit OASIS documentation within established timelines
Preferred Experience / Skills
- Experience supervising Physical Therapy Assistants in a home health setting
- Familiarity with home health documentation systems and care coordination workflows
