Key Responsibilities
- Provide standard physical therapy services to a home health patient population of all ages with multiple comorbidities
- Manage a caseload of approximately 4–5 patients per 8-hour day
- Complete all clinical documentation within 1 day of each patient visit
- Utilize HCHB (HomeCare HomeBase) EMR system for documentation and patient management
- Travel to patient homes; mileage is reimbursable per GSA rate
- Collaborate with support staff including COTAs and coordinate with a supervisor available 24/7
Required Qualifications
- Active California Physical Therapist (PT) license
- BLS certification required
- Valid driver's license and current auto insurance
- Prior home health experience required
- All required onboarding modules must be completed prior to start date — no exceptions
Preferred Experience / Skills
- Experience treating patients with complex, multiple comorbidities across all age groups
- Familiarity with HCHB (HomeCare HomeBase) EMR platform
- Strong time management and documentation skills in a field-based setting
