    Contract | Days | Monday–Friday (Mondays longer, Fridays slightly shorter)

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Maintain clear, concise, and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill continuing education requirements
    • Actively participate in performance improvement initiatives and contribute feedback to maximize quality of care outcomes
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of Tennessee
    • Minimum of 1 year of outpatient physical therapy experience
    • Ability to complete documentation and ensure care is provided under valid physician orders and within insurance approvals
    • Commitment to patient and employee safety standards and safe patient handling practices
    Preferred Experience / Skills
    • Dry needling certification or experience
    • Experience developing individualized professional development plans and pursuing continuing education proactively
    • Familiarity with performance improvement methodologies in a clinical setting
