Key Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
- Meet established department objectives while delivering high-quality patient care
- Serve as co-leader in an interdisciplinary approach to patient groups
- Provide home health physical therapy services, including travel to patient locations
Required Qualifications
- Doctor of Physical Therapy (DPT) degree required if license was obtained on or after January 2016, or foreign equivalent
- Active Physical Therapy licensure in all applicable states of practice
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor
- Valid Driver's License required; must be at least 21 years of age or older
Preferred Experience / Skills
- Prior experience in a home health or community-based physical therapy setting
- Experience working within an interdisciplinary care team environment
