Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established goals
• Collaborate with interdisciplinary care team to support patient outcomes

Required Qualifications

• Active Physical Therapist (PT) license in the state of Maryland
• Graduate of an accredited Physical Therapy program
• Prior experience working in a skilled nursing or long-term care setting

Preferred Experience / Skills

• Experience with geriatric patient populations
• Familiarity with MDS documentation and Medicare guidelines in SNF environments
