Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
- Develop and implement individualized physical therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and caregivers on therapeutic exercises and home programs to support recovery
Required Qualifications
- Active Physical Therapist (PT) license in the state of Massachusetts
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility (SNF) setting
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior travel or contract therapy experience
- Familiarity with SNF documentation and Medicare billing requirements
