Key Responsibilities
- Provide direct physical therapy services to patients in an outpatient setting
- Complete required clinical documentation accurately and in a timely manner
- Develop and implement individualized treatment plans based on patient evaluations
- Collaborate with the interdisciplinary care team to support patient progress and outcomes
Required Qualifications
- Active Physical Therapist license in the state of Oregon (permanent licensure required prior to assignment start)
- Experience in an outpatient physical therapy setting
- Ability to meet contract credentialing and quality assurance requirements prior to start
Preferred Experience / Skills
- Prior experience working in a high-volume outpatient clinic environment
- Strong documentation skills and familiarity with electronic medical records
