Key Responsibilities Conduct skilled home health visits, completing a minimum of 6–8 patient visits per day across a diverse caseload including med/surg, cardiac, ortho, neuro, post-surgical, COPD, CHF, CVA, Parkinson's, and trauma patients. Key Responsibilities

• Conduct skilled home health visits, completing a minimum of 6–8 patient visits per day across a diverse caseload including med/surg, cardiac, ortho, neuro, post-surgical, COPD, CHF, CVA, Parkinson's, and trauma patients
• Perform comprehensive patient assessments including cardiopulmonary/circulatory evaluation, safety and injury prevention, and signs of physical, sexual, and psychological abuse
• Develop and implement Home Exercise Programs and provide patient and family education
• Utilize and assess equipment and devices including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, wheelchairs, upper extremity prosthetics, and supplemental oxygen
• Complete accurate and timely clinical documentation, billing, and scheduling using facility-designated systems and portable technology provided
• Participate in weekend coverage as required (1–2 weekends per contract)

Required Qualifications

• Active Florida PT license or valid compact license required at time of submission
• BLS certification required
• Minimum 1 year of physical therapy experience required
• Home health experience required
• Valid driver's license and reliable personal vehicle required for home visits
• COVID-19 vaccination required (or approved exemption on file); flu shot or declination required with mandatory mask wearing if declined
• Must live at least 100 miles from the facility to be eligible

Preferred Experience / Skills

• OASIS training preferred
• Experience with Medicare, Medicare Advantage, and commercial insurance skilled home care
• Cerner charting experience preferred
