PT - Physical Therapist
Job Details:
• Day shift schedule from 8:00 AM to 5:00 PM
• Full-time position
• Current New Mexico Physical Therapist Licensure required
• BLS certification via AHA required
• Bachelor's, Master's, or entry level Doctorate of Physical Therapy degree from an accredited college or university required
• Perform comprehensive diagnostic evaluations to determine physical and cognitive abilities of inpatients and outpatients
• Develop and implement individualized treatment plans with measurable goals in collaboration with patients and families
• Maintain accurate documentation including initial patient evaluations, daily and weekly progress notes, discharge summaries, and other notations as appropriate
• Participate in patient care conferences, family conferences, in-service programs, and departmental meetings
• Counsel patients, families, and other involved individuals regarding patient evaluation, care, and treatment
• Initiate and complete discharge planning in a timely manner and provide effective patient education and training
• Assist with coordination and supervision of Physical Therapy and Physical Therapy Assistant student programs as well as the volunteer program
• Assist with clinical aspects of the department including orientation of new associates and scheduling of patients
• Supervise Licensed Physical Therapy Assistants, Rehab Technicians, and Volunteers
• Support and participate in Performance Improvement activities
• Maintain awareness of community agencies to make appropriate referrals and increase knowledge of community resources
• Proficiency with all modalities and equipment used by a Physical Therapist as outlined by the Executive Council of Physical Therapy and Occupational Therapy Examiners required
