Key Responsibilities
- Provide skilled physical therapy services to patients in a skilled nursing facility setting
- Evaluate, develop, and implement individualized treatment plans to support patient rehabilitation and functional goals
- Document patient progress and maintain accurate clinical records in compliance with facility and regulatory standards
- Must be open to floating between worksites as needed during scheduled shifts
Required Qualifications
- Active California Physical Therapist (PT) license required
- Minimum 1 year of SNF experience required
- Standard credentialing requirements must be met prior to start
- Ability and willingness to float as operationally needed
Preferred Experience / Skills
- Experience with Casamba documentation software strongly preferred
- Familiarity with skilled nursing facility workflows and interdisciplinary care teams
