Key Responsibilities
- Provide skilled physical therapy evaluation and treatment services to patients in a skilled nursing facility setting
- Develop and implement individualized treatment plans in accordance with physician orders and patient goals
- Document patient progress, treatment plans, and outcomes using facility EMR systems
- Collaborate with interdisciplinary care team to ensure coordinated patient care
- Must be open to floating between assigned worksites as needed
Required Qualifications
- Active California Physical Therapist (PT) license required
- Minimum 1 year of SNF (Skilled Nursing Facility) experience required
- Standard credentialing requirements must be met prior to start
Preferred Experience / Skills
- Experience with Casamba EMR software highly preferred
- Strong documentation and communication skills within a multidisciplinary team environment
