Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting
- Evaluate, develop, and implement individualized patient care plans
- Supervise and oversee Physical Therapist Assistants (PTAs)
- Document patient progress and clinical notes using HCHB EMR
- Participate in a 2-day in-office orientation at the start of the assignment
Required Qualifications
- Active Physical Therapist (PT) license in the state of Texas
- Experience in a home health setting
- Must reside within 40 miles of the facility to be eligible
- No on-call or weekend availability required
Preferred Experience / Skills
- Prior experience with HCHB electronic medical records system
- Experience supervising or mentoring PTAs
- Strong time management and independent practice skills in a field-based environment
