Key Responsibilities
Perform comprehensive assessments of assigned patients, including OASIS and PT evaluations, encompassing psychosocial, educational, safety, and risk factor parameters in accordance with federal and state regulations and APTA guidelines
Develop and implement individualized plans of care with patient-specific goals, providing skilled physical therapy treatment including therapeutic techniques and patient/caregiver education
Coordinate patient care as Care Manager and/or team member with physicians, nurses, caregivers, and other rehabilitation services to ensure optimal outcomes
Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations and APTA guidelines
Complete OASIS time points, clinical documentation, and billing/charging paperwork accurately and in compliance with agency, state, and federal guidelines
Actively monitor and manage patient visit schedules to maximize productivity, communicating changes to schedulers and supervisors in a timely manner
Required Qualifications
Active Physical Therapist license in the state of Vermont (or eligibility to obtain)
Knowledge of and ability to comply with federal and state home health regulations, including OASIS documentation requirements
Proficiency in completing required agency, state, and federal forms (e.g., HHABN, Medicare Notice of Non-Coverage)
Ability to perform patient-related health assessments and procedures, including wound care and staple/suture removal
Preferred Experience / Skills
Prior experience in a home health setting with OASIS documentation
Familiarity with McKesson home health software processes
Experience supervising PTAs, PT students, or home care aides
Background in quality improvement collaboration and peer review participation
