    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
    • Conduct 60-minute evaluations and treatment sessions, designing functional treatment programs to support effective return to community activities
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals
    • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act and regulatory agency requirements while actively pursuing continuing education and professional development
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of New Jersey
    • Compliance with all state-mandated continuing education requirements
    • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
    • Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
    • Demonstrated commitment to patient safety and safe patient handling practices
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Familiarity with performance improvement methodologies and quality of care initiatives
