Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care teams to support patient outcomes
- May be required to float to an additional facility location within the region
Required Qualifications
- Active Physical Therapist (PT) license in the state of Pennsylvania
- Graduate of an accredited Physical Therapy program
- Ability to work day shift hours
- Willingness and ability to float to a secondary facility location as needed
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care setting
- Experience with geriatric patient populations
- Familiarity with electronic medical records and therapy documentation systems
