Contract | Days (8:30 AM – 5:00 PM) | Field-Based / Home Health
Key Responsibilities
Provide physical therapy services to patients in home environments, assisted living facilities, senior apartments, and board and care homes
Manage a caseload of 3 to 5 patients per day, including up to 2 Start of Care visits or a combination of evaluations and follow-up visits
Treat a varied patient population across diagnoses including orthopedic, cardiopulmonary, oncology, neurological, geriatric, and amputation cases (no pediatrics or hospice)
Complete OASIS assessments, clinical documentation, and case management responsibilities within the assigned shift
Supervise up to 2 PTAs or COTAs as part of the home health team
Participate in a structured 3-week orientation including online learning and supervised field precepting prior to independent practice
Required Qualifications
Active California Physical Therapist (PT) license
Current BLS certification
Active National Provider Identifier (NPI) number
Minimum 2 years of experience in a healthcare setting
OASIS competency and home health experience required
Valid driver's license and current proof of automobile insurance
Preferred Experience / Skills
Experience with HomeCare HomeBase (HCHB) EMR system
Community or home health experience strongly preferred
Strong oral and written communication skills with solid organizational ability
Ability to work comfortably in varied home environments, including those that may be physically challenging
