Key Responsibilities
Provide direct physical therapy services to patients in an outpatient setting
Complete required documentation in accordance with facility and contractual standards
Deliver evidence-based treatment plans tailored to individual patient needs and goals
Collaborate with interdisciplinary team members to support patient progress and outcomes
Required Qualifications
Active Physical Therapist license in the state of Wisconsin (compact licensure may be accepted initially, but permanent Wisconsin licensure must be obtained promptly)
Experience in an outpatient physical therapy setting
Ability to meet facility credentialing and contract QA requirements prior to assignment start
Candidates who have worked permanently or PRN at this facility within the past year are not eligible for this assignment
Preferred Experience / Skills
Prior travel therapy experience
Strong documentation skills and ability to adapt to facility-specific systems
