Key Responsibilities
- Provide direct physical therapy services to patients in an outpatient setting
- Evaluate, assess, and develop individualized treatment plans based on patient needs
- Deliver therapeutic interventions and monitor patient progress toward goals
- Complete required clinical documentation accurately and in a timely manner
- Collaborate with the care team to ensure continuity and quality of patient care
Required Qualifications
- Active Physical Therapist license required (Oregon permanent license required; compact license may be accepted at start but permanent Oregon licensure must be obtained promptly)
- Experience in an outpatient physical therapy setting
- Ability to fulfill contract credentialing and quality assurance requirements prior to assignment start
Preferred Experience / Skills
- Prior experience working in a high-volume outpatient therapy environment
- Strong documentation skills and familiarity with electronic medical records
