Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM | Onsite
Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting across an assigned territory
- Evaluate patients and develop individualized treatment plans to address functional goals
- Deliver and document therapy services utilizing the HCHB (HomeCare HomeBase) EMR platform
- Meet expected productivity of 30 points per week (approximately 6 points per day)
- Coordinate care with interdisciplinary team members to support patient outcomes
Required Qualifications
- Active Physical Therapist (PT) license in the state of Oregon
- Previous home health (HH) experience required
- Previous experience with HCHB (HomeCare HomeBase) required
- Ability to travel within a 50-mile radius of the office location
Preferred Experience / Skills
- Strong time management and self-directed scheduling skills suited for a field-based role
- Experience managing a high-volume caseload in a home health environment
