Key Responsibilities
Provide direct physical therapy services to patients in an outpatient or clinical setting
Evaluate, assess, and develop individualized treatment plans for patients
Implement therapeutic interventions and monitor patient progress toward goals
Complete required clinical documentation accurately and in a timely manner
Collaborate with interdisciplinary team members to ensure continuity of patient care
Required Qualifications
Active Physical Therapist license in the state of Indiana (permanent state license required)
Prior experience working in a physical therapy clinical setting
Ability to meet credentialing and contract quality assurance requirements prior to start
Must not have worked at this facility in a permanent or PRN capacity within the past year
Preferred Experience / Skills
Experience working in a high-volume outpatient physical therapy environment
Familiarity with travel or contract therapy assignments and associated documentation processes
