Key Responsibilities
Provide physical therapy services across two assisted living facilities
Evaluate and treat residents to improve mobility, strength, and functional independence
Develop and implement individualized treatment plans in accordance with care goals
Collaborate with interdisciplinary care teams to support resident outcomes
Travel between two facility locations as part of a split assignment
Required Qualifications
Active Physical Therapist (PT) license in the state of Wisconsin
Ability and willingness to travel between two facility locations regularly
Strong clinical assessment and documentation skills
Preferred Experience / Skills
Prior experience working in an assisted living or long-term care setting
Experience managing a caseload across multiple sites
