    Key Responsibilities
    • Provide direct physical therapy services to home health patients in accordance with each patient's individualized plan of care
    • Collaborate with interdisciplinary team members to develop and implement comprehensive patient care plans
    • Evaluate and recommend rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
    • Apply appropriate therapeutic modalities as indicated by patient need
    • Complete and maintain accurate clinical documentation, including proficiency with OASIS assessments
    • Work independently in a home health setting with minimal supervision
    Required Qualifications
    • Active Massachusetts Physical Therapist License — must be provided at time of submission
    • Active American Heart Association BLS Certification
    • Home Health Certification required
    • Certified home health experience with OASIS proficiency — required and non-negotiable
    • Minimum 2+ years of acute nursing care (hospital-based) and home care experience
    • Valid driver's license and personal vehicle required
    • Must be able to pass an N95 respirator fit test (PAPRs are not available for this setting)
    • Must reside more than 50 miles from the facility to be eligible
    Preferred Experience / Skills
    • Acute care experience in a hospital-based setting
    • Ability to maintain a commute of 45 minutes or less from the designated office location
