    Contract | Days | Monday – Friday with two weekend days required per month

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary team, focused on progressing patients toward discharge within an established length of stay
    • Delegate and direct discipline-specific therapy services for assigned patients, managing individual, concurrent, and group treatment sessions based on patient care needs
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines
    • Coordinate discharge planning efforts including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act, maintain professional licensure in good standing, and fulfill all state-mandated continuing education requirements
    Required Qualifications
    • Active Physical Therapist license in good standing (or eligibility to practice in Arizona)
    • Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
    • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
    • Ability to work Monday – Friday with two weekend days per month
    • Commitment to patient and employee safety standards and organizational compliance expectations
    Preferred Experience / Skills
    • Experience in an inpatient rehabilitation or acute care setting
    • Familiarity with interdisciplinary team collaboration and discharge planning processes
    • Experience supervising or mentoring PT students or clinical affiliates
    • Active involvement in professional development, continuing education, or professional organizations
