Key Responsibilities

• Evaluate and treat patients in accordance with physical therapy standards of practice within the assigned outpatient setting
• Develop and implement individualized treatment plans to support patient recovery and functional goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with the interdisciplinary care team to ensure continuity and quality of patient care
• Maintain compliance with facility credentialing and documentation requirements throughout the assignment

Required Qualifications

• Active Physical Therapist license (must be valid and in good standing at time of submission)
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility credentialing requirements prior to start
• Must provide proof of license application within 5 business days of booking if permanent licensure is pending
