Key Responsibilities
- Provide skilled physical therapy evaluation and treatment services to patients in a skilled nursing facility setting
- Develop and implement individualized treatment plans in accordance with patient needs and clinical goals
- Document patient progress and maintain accurate clinical records; experience with Casamba EMR is highly preferred
- Must be open to floating between worksites as operationally required
- Collaborate with interdisciplinary care team to support optimal patient outcomes
Required Qualifications
- Active California Physical Therapist (PT) license required
- Minimum of 1 year of SNF (skilled nursing facility) experience required
- Standard credentialing requirements must be met prior to start
- Ability to work day shift hours and float as needed
Preferred Experience / Skills
- Experience with Casamba EMR documentation system strongly preferred
- Prior experience floating between multiple care sites
