Contract | Monday–Friday, 8:00 AM–5:00 PM (Days) | 14-Week Assignment
Key Responsibilities
- Conduct physical therapy evaluations and provide subsequent visits for home health patients within an approximately 40-mile coverage territory
- Expected caseload of 2 evaluations and 3–4 subsequent visits, or 5–6 total visits per day
- Oversee and supervise the work of Physical Therapist Assistants (PTAs) in the field
- Document all patient care and clinical notes using the HCHB EMR system
- Complete a minimum of 2 days of in-office orientation during the first 2 weeks of the assignment
Required Qualifications
- Active Physical Therapist (PT) license in the state of California
- Experience in a home health setting
- Ability to travel within an approximately 40-mile service radius
- Proficiency with or ability to learn the HCHB electronic medical records system
- Valid driver's license and reliable transportation required for field-based visits
Preferred Experience / Skills
- Prior experience supervising or collaborating with Physical Therapist Assistants (PTAs)
- Familiarity with home health documentation standards and productivity expectations
- Strong time management skills to effectively manage a daily visit caseload
