Key Responsibilities
- Assess patient conditions and develop and implement individualized care plans in a home health setting
- Provide ongoing education and support to patients and their families throughout the care process
- Perform admission processes in accordance with organizational policies and standards of practice
- Carry out case management responsibilities, including interdisciplinary communications and coordination of care
- Maintain thorough and timely electronic documentation in compliance with organizational standards
- Travel to assigned patient cases and participate in weekend rotations on a scheduled basis
Required Qualifications
- Active Physical Therapist license in the state of Massachusetts
- Minimum of 1 year of experience in a Certified Home Health Agency (CHHA)
- Minimum of 1 year of Case Management experience within a certified home health setting
- Experience in hospice, private duty, or standalone case management roles does not satisfy the certified home health requirement
- Ability to travel to patient cases as assigned
Preferred Experience / Skills
- Familiarity with electronic medical records (EMR) systems used in home health environments
- Strong interdisciplinary communication and care coordination skills
