Key Responsibilities
- Provide physical therapy services to patients within a home health setting across an assigned territory
- Manage a caseload with an expected productivity of 30 points per week (approximately 6 points per day)
- Conduct evaluations, develop individualized treatment plans, and deliver skilled PT interventions in the patient's home
- Document patient care accurately and in a timely manner using HCHB (Homecare Homebase) software
- Coordinate with the interdisciplinary care team to ensure continuity and quality of patient care
Required Qualifications
- Active Physical Therapist (PT) license in the state of Oregon
- Previous home health experience required
- Previous HCHB (Homecare Homebase) experience required
- Must reside within or be able to service a 50-mile radius from the office location
Preferred Experience / Skills
- Strong time management and self-directed work skills suited for an independent home health environment
- Experience managing productivity standards in a home health or field-based therapy role
