Key Responsibilities
Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation, modalities, use of supportive and assistive devices, and wound care
Assess and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes for effectiveness in the electronic documentation system
Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed
Required Qualifications
Active Oklahoma State Physical Therapist license
Minimum of 1 year of physical therapy experience, or new graduate with completed Level II clinical experience
Experience in long-term care (LTC) or rehabilitation setting
Adherence to APTA and Oklahoma state practice acts, code of conduct, and professional ethics
Preferred Experience / Skills
Experience with standardized assessments and evidence-based clinical interventions
Familiarity with electronic documentation systems used in clinical or long-term care environments
