    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
    • Conduct 60-minute evaluation and treatment sessions in an outpatient setting
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
    • Supervise and mentor students as assigned, providing a positive learning environment including development of learning objectives and completion of student evaluations
    • Adhere to discipline-specific state practice acts and regulatory requirements, and participate in organizational performance improvement initiatives
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Colorado
    • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
    • Proficiency in clinical documentation in compliance with state, federal, and payer guidelines
    • Knowledge of and adherence to discipline-specific state practice acts and professional codes of conduct
    • Commitment to patient and employee safety standards and safe patient handling practices
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Active involvement in continuing education and professional development activities
