    Contract | 13 Weeks | Days | Monday–Friday, 8 hours/day

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Ensure care is provided under signed, valid physician orders and within insurance approvals as required for clinical care
    • Delegate and direct all discipline-specific therapy services for assigned patients, designing functional treatment programs to support effective return to community activities
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
    • Adhere to discipline-specific State Practice Act and all regulatory agency requirements, maintaining professional licensure in good standing
    Required Qualifications
    • Active Physical Therapist license in good standing in the state of Tennessee
    • Minimum 1–2 years of outpatient physical therapy experience
    • Completion of all state-mandated continuing education requirements
    • Ability to perform in an ethical manner in accordance with discipline-specific professional code of conduct
    Preferred Experience / Skills
    • Experience with functional quality indicator assessments and letters of medical necessity
    • Active involvement in professional development activities, continuing education, or professional organizations
    • Familiarity with performance improvement methodologies and quality improvement initiatives
