Key Responsibilities
Provide physical therapy evaluation and treatment services to residents in a skilled nursing facility setting
Develop and implement individualized treatment plans to maximize patient functional outcomes
Travel to an additional skilled nursing facility location in the region as needed
Collaborate with interdisciplinary care teams to support resident rehabilitation goals
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
Active Physical Therapist (PT) license in the state of Wisconsin
Willingness and ability to travel to an additional facility location within the region
Graduate of an accredited Physical Therapy program
Preferred Experience / Skills
Prior experience working in a skilled nursing facility or long-term care setting
Experience with geriatric patient populations
Strong communication and interdisciplinary team collaboration skills
