Key Responsibilities
- Provide skilled physical therapy services to home health patients in Medford, OR and surrounding areas
- Conduct patient evaluations and develop individualized treatment plans in a 1:1 care setting
- Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
- Complete clinical documentation using EPIC and OASIS
- Coordinate patient groupings to minimize drive time, with occasional travel radius of 50+ miles
Required Qualifications
- Minimum of 1 year of skilled home health physical therapy experience required
- Active or pending Physical Therapist license in the state of Oregon
- Basic Cardiac Life Support (BCLS) certification required
- OASIS documentation experience required
- Reliable personal transportation required
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills
- Familiarity with EPIC electronic health records system
- Experience meeting productivity point expectations in a home health environment
- Comfort with variable daily scheduling and independent field-based work
