Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM | Onsite/Field
Key Responsibilities
- Provide physical therapy services to patients within a home health setting across an assigned territory
- Manage a caseload with an expected productivity of 30 points per week (6 points per day)
- Conduct evaluations, develop individualized treatment plans, and deliver skilled PT interventions in the home environment
- Document patient care accurately and in a timely manner using HCHB (HomeCare HomeBase) electronic medical records
- Collaborate with interdisciplinary team members to coordinate patient care and progress toward goals
Required Qualifications
- Active Physical Therapist (PT) license in the state of Oregon (or eligibility to obtain)
- Previous home health experience required
- Previous experience with HCHB (HomeCare HomeBase) documentation system required
- Ability to travel within a 50-mile radius of the office
Preferred Experience / Skills
- Strong time management and self-directed work skills suited for a field-based role
- Experience managing productivity standards in a home health environment
