Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Deliver skilled physical therapy services to residents within a skilled nursing facility setting
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to support patient recovery and functional goals
- Educate patients and family members on therapeutic exercises and home programs as appropriate
Required Qualifications
- Active Physical Therapist (PT) license in the state of Ohio
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior experience working in a skilled nursing facility or long-term care environment
- Familiarity with MDS documentation and Medicare guidelines
