Key Responsibilities
- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to support patient outcomes
- Ensure compliance with facility policies and applicable regulatory standards
Required Qualifications
- Active Physical Therapist (PT) license in the state of Wisconsin
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a clinical or rehabilitation setting
Preferred Experience / Skills
- Prior experience in a Skilled Nursing Facility (SNF) environment
- Familiarity with Medicare documentation and compliance requirements
