Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Manage a productivity caseload of 28 points per week
• Oversee and supervise Physical Therapist Assistants (PTAs)
• Utilize Home Care Home Base (HCHB) software for documentation and care coordination
• Travel to patient homes within the designated service area

Required Qualifications

• Active Physical Therapist (PT) license in the state of Connecticut
• Prior home health experience required
• Home Care Home Base (HCHB) experience required
• Must reside within approximately 40 miles of the Mansfield or Norwich area to be eligible

Preferred Experience / Skills

• Experience supervising or overseeing PTAs in a clinical setting
• Strong time management and self-directed work skills suited for a home health environment
