    Key Responsibilities
    • Provide physical therapy services to patients in a home health setting in accordance with physician written orders
    • Interact regularly with patients, family members, agency personnel, and physicians to coordinate and deliver careli>Demonstrate and apply knowledge of state therapy laws, Federal Conditions of Participation, and applicable home health regulatory requirements
    • Serve as a clinical education coordinator, supporting student education, mentorship, in-service coordination, and performance improvement initiatives
    • Identify and address safety issues for staff and patients across all care settings
    • Assess patient data reflective of age-specific needs and provide appropriate, individualized care across the lifespan
    Required Qualifications
    • Active Physical Therapist license in accordance with applicable state requirements
    • Knowledge of home health regulations, including Federal Conditions of Participation for Home Health Agencies
    • Ability to perform moderate physical activity including walking, standing, bending, lifting, and carrying objects up to 50 pounds
    • Strong interpersonal, communication, and organizational skills with the ability to work independently and manage time effectively
    • Valid driver's license and ability to travel to patient homes within the service area
    Preferred Experience / Skills
    • Prior experience in a home health or community-based therapy setting
    • Experience with clinical preceptorship, student education, or performance improvement activities
    • Familiarity with community resources and referral systems relevant to home health patient populations
