Key Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily patient visits in a home health setting
• Manage a caseload of approximately 6.5 points per day within a home health territory
• Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
• Participate in rotating weekend shifts and holiday rotation as scheduled
• Utilize Home Care Home Base (HCHB) software for documentation and patient management

Required Qualifications

• Active, unrestricted Physical Therapist license in hand at time of submission
• Current BLS certification
• Demonstrated experience with Home Care Home Base (HCHB) software
• Prior home health experience required

Preferred Experience / Skills

• Experience treating patients with CHF, orthopedic diagnoses, or pneumonia in a home health setting
• Candidates with prior travel or contract home health experience preferred; new graduates are discouraged from applying
